I can't say it "never" happens, as I do hear of a case every now and then. It was a much bigger issue on the QR25DE engines used in the 02-03 Altimas and Sentras to the point where they had to recall them.

The O2 spacers on the rear sensors help with the catalyst efficiency codes from triggering, but it will have no effect on the lean condition nor the "system lean" codes that may occur.
