Serpentine belt and engine clicking noise

2008 Pathfinder, V6, 110,000 miles. Had plugs, belt, oil changed 2 months ago and vehicle ran fine. 3 days ago started her up and heard a loud squeal from the belt and lost power steering. Popped hood and found the belt had come off and just hanging. Put belt back on and the next day heard a cliking noise coming from the engine. Noise gets more rapid as engine speed increases. Removed belt, started engine and still heard ticking noise which seems to move from left side of engine to right side and then sometimes just quits. What could cause the belt to actually come off,(There were no sticks, animals, etc under the hood) and any idea what the ticking noise could be? Thanks in advance.

Shop could have used the wrong belt, coupled with a bad tensioner could have caused the belt to jump maybe.

I'd check the tensioner to see if it needs doing, and also the alignment of the ps pump. Do you know which belt was used (part number too so we can tell if it's the right length).
