Gas Cap Separation

Has anyone experienced an issue where the gas cap stopper separated from the top of the cap? I cant see where anything might have cracked and cant figure how the two were originally attached, unless I'm missing a piece.

I am also facing the same issue here. If someone could help with an answer it would be great.

Ya know, since I first posted this, I somehow put it back together and it hasn't given me a problem since. I'm just careful when I unscrew the cap to make sure everything stays together.
